St. Croix Hospice started in Minneapolis around 12 years ago by Heath Bartness. He was an Executive Director at a nursing home that decided to build his own hospice company as he felt that the current agencies he was utilizing were not delivering the highest quality of care. We are now in ten states and serve 4,500 patients nationwide. The Illinois territory covers all the Chicagoland area and suburbs. We have branches located in Des Plaines, Lombard, and Rockford.
Hospice care improves comfort and quality of life for patients with a terminal diagnosis. Hospice care is a benefit covered by Medicare/Medicaid and most private insurance. It is provided wherever patients call home.
The hospice care we provide includes a personalized care plan to meet each patient’s individual needs and wishes. Our team includes physician, nursing and aide care wherever the patient calls home. The goal of our care is to provide pain and symptom management for improved quality of life. We provide coverage for medication, supplies and equipment related to the terminal diagnosis. In addition, we provide expert specialized care provided by trained staff through our specialty care programs such as spiritual, social and emotional support for both patients and their loved ones.

North Star Dementia: Our clinical staff are certified Dementia Care Specialists positioning us as a stronger hospice partner for facilities that specialize in memory care.
Bridge Care: If we admit a patient on a Friday. Our staff will visit the patient on Saturday, Sunday, and Monday to ensure that the patient and their family’s needs are met over the weekend.
Navigate Falls: Each patient is initially assessed for fall risk factors. If they are deemed a high fall risk, St. Croix Hospice will contract an outside PT agency to do a one-time home safety assessment and provide family with fall prevention measures, exercises, and equipment.
